What You'll Do: 

We are looking for volunteers to support the connecting communities  team with HMP Holme House, Stockton. The right person will support with service delivery, supporting service users who access the service. Duties will include, Carrying out comprehensive assessments, triage, risk assessments, risk management plans and to contribute to the development of individuals community recovery plans. To assist project workers with POD and group facilitation. Review ongoing care and treatment whilst liaising closely with full range of specialist worker within a multi-disciplinary team. Support project workers on home visits. Contribute to the overall performance of the service to ensure that output targets are achieved. ensure that effective case management and coordination, covering areas of risk with a particular focus on safeguarding children and adults, problematic behaviour, housing social and family functioning, and psychological and physical and mental health.
