We are recruiting Discharge Support Volunteers and Volunteer Responders to join our growing volunteer team.

What You'll Do: 

Discharge Support Volunteers will work under the supervision of staff to provide a service which supports all discharge functions, including supporting staff, patients and family whilst maintaining respect for individuality, confidentiality and the dignity of all patients. 

The role can also include supporting patients after discharge for a limited time to ensure they reintegrate back into their community and to help support them during those first few vital days/weeks.

The role will include:

 

  • Meeting patients whilst on a ward to develop relationship upon discharge.
     
  • Liaison with staff on the ward to identify any potential referrals to the service.

 

  • Discuss with patients, their potential support needs upon discharge home i.e. transport or clothing. 

 

  • As necessary, arranging with staff, transport via volunteer drivers.

 

  • Liaison with staff from the Integrated Discharge Team for additional support.

 

  • Maintaining contact and support with discharged patients for 28 days either by visits to home or by phone to ensure they are integrated into the community.

 

  • Being an ambassador of the Trust at all times.

 

Volunteer Responders will provide support to services and departments with activities that would normally take staff away from their area of work i.e. delivering samples to the lab or delivering patient belongings to the ward.  Responder volunteers will answer calls via vocera and will receive training on how to use this device.
